Test Irregularity Documentation for CTE EOP Assessment
As a part of state accountability, it is necessary to document and report all testing irregularities. Please provide the following information as documentation of this testing irregularity. All test irregularities must also be submitted to the District Assessment Coordinator. Note: Please do not submit student name, instead provide SSID only.

2. Who was proctoring the test?
3. When was the issue reported by the proctor to the test coordinator/administrator?
4. When was the issue reported to OCTE?
5. Which student(s) were involved? Please do NOT provide any personal identification information other than SSID.
6. Which assessment(s) were the student(s) taking?
7. Provide the approximate start time for each test taker.
8. Describe the irregularity in testing that prompted request for assistance.
